Coordinate specialty patient care services

Educate patients and caregivers

Coordinate follow-up care activities

Communicate with providers and care teams

Monitor patient outcomes

Maintain clinical documentation

Support quality and safety initiatives

What You'll Bring

Required Qualifications

Current RN licensure

Strong clinical assessment skills

Effective communication abilities

Ability to work independently

Preferred Qualities

Wound care experience

Cardiology experience

Vascular access experience

Specialty certification
